Maywood Dial-A-Ride Program - 2013
Effective January 2, 2013, the City of Maywood will no longer provide Dial-A-Ride service to the general public. Dial-A-Ride service will only be available to City of Maywood residents 62 years and over or those with a physician-certified disability which prohibits the use of public transportation. Individuals requiring special assistance will be permitted to have an attendant accompany them. All residents wishing to continue using the Dial-A-Ride service under the new guidelines must complete a City of Maywood Dial-A-Ride application and be issued a picture ID card prior to January 2, 2013. Applications are being accepted at the front office of the Maywood Activity Center – 4801 E. 58th Street, Monday thru Thursday from 7:30 a.m. to 6:30 p.m. Friday from 7:30 a.m. to 3:30 p.m. You must bring a state issued ID and a utilities bill. PLEASE NOTE THE CURRENT GREEN MAYWOOD AREA TRANSIT RIDER PASS CARD WILL NO LONGER BE ACCEPTED BEGINNING JANUARY 2, 2013. |
